COACH EVALUATION FORM: This form is used to aid our Soccer Board in finding the best coaches for you and our children. Any comments on this form are for the Boards use only. Please take the time to fill it out. NO coach will see his/her forms (Head Coaches will not see evaluations from his teams).
CONSENT FOR MEDICAL TREATMENT: As the parent or legal guardian of:__________________________________, I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care can be given under whatever conditions are necessary to preserve the life, limb or well being of my dependant.
GTCSC PLAYER DROP AFTER DRAFT: Any player who withdraws after the draft is NOT entitled to a refund. However, the following process must be followed to officially drop a player from a team roster.
GTCSC PLAYER DROP PRIOR TO DRAFT: Any player who withdraws prior to the draft shall be entitled to a full refund of their registration fee.
INTENT TO COACH: I do hereby acknowledge my intention to coach in the age group and division of (Under) U-____ with Grand Terrace Community Soccer Club. I understand that I must meet the qualifications and requirements of CYSA Risk Management before the season begins.
TEAM TROPHY LIST: Must be turned into the league secretary in secretary's box at the board member area near the snack bar.
INCIDENT FORM: Hopefully you don't need this one.
PROGRAM ADMINISTRATOR REGISTRATION FORM: Program Administrators are defined as: State Board, Coaching Education staff, O.D.P. coaches and scouts, district, league and club officers, program directors, team managers, athletic trainers, coaches, assistant coaches, and substitute coaches, and anyone who has an official capacity in the soccer program.
